Terms of Service

Agreement to Terms

By accessing or using the services of Intention Therapy & Coaching, you agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use our services.

Services Provided

Dustyn Salomon is an Associate Marriage and Family Therapist (AMFT #147702) supervised by Heena Parikh, LMFT #47321, providing:

• Therapy Services: Individual and couples therapy for California residents
• Coaching Services: Dating and relationship coaching (not therapy)

Coaching services are not a substitute for therapy, medical treatment, or mental health services. If you are experiencing a mental health crisis, please contact emergency services or a crisis hotline.

Eligibility

Therapy services are available only to clients located in California at the time of service. Coaching services may be available to clients in other locations. You must be 18 years or older to use our services, or have parental/guardian consent.

Appointments & Cancellations

Please adhere to the following policies:

• Appointments are scheduled through SimplePractice or by contacting us directly
• Please provide at least 24 hours notice for cancellations
• Late cancellations or missed appointments may be subject to a fee
• Sessions start at the scheduled time; late arrivals will not extend the session

Payment Terms

Payment policies include:

• Therapy: Contact us for rates. Insurance may be accepted through our group practice (Cigna & Aetna)
• Coaching: $150 per 55-minute session. Coaching is not covered by insurance
• Payment is due at the time of service
• Accepted payment methods: Credit/Debit cards, HSA/FSA, Venmo, Zelle

Telehealth Services

Services are provided via secure telehealth platforms. By using telehealth services, you acknowledge that while we use secure, encrypted platforms, there are inherent risks with electronic communication. You agree to be in a private, safe location during sessions.

Limitations of Liability

Therapy and coaching outcomes vary by individual. We do not guarantee specific results. The information on this website is for general informational purposes only and does not constitute professional advice or create a therapist-client relationship.

Termination of Services

Either party may terminate the therapeutic or coaching relationship at any time. If services are terminated, appropriate referrals will be provided upon request.

Changes to Terms

We reserve the right to modify these terms at any time. Changes will be posted on this page with an updated revision date. Continued use of our services constitutes acceptance of any changes.
